Parade Entry Sign Up Form

This will be used to announce your entry in the parade
Will your entry be participating in another parade the same day?(Required)
Will your entry have music playing?(Required)
Will your entry be distributing something? (Candy, flyers, etc.?)(Required)
I agree to comply with all West Bountiful City safety rules and regulations.(Required)
I or a representative of my entry agree to attend the mandatory safety meeting on either June 26th or 30th. I understand that if I do not attend, I will be ineligible to participate in the parade.(Required)
